    Form of Undertaking

In the unlikely case of a serious ill-health or other emergency situation in which you are not able to give your own consent, we need you to let us know whom you would like to make decisions on your behalf. We need them to complete this form so that they are aware of this responsibility. We would normally try to contact the person you consider to be your next of kin wherever they live, but we do need a contact who is living in the UK. If you have no close relatives or friends in the UK who would fulfil this role for you, it is possible for you to authorise the College nurse or Head of Wellbeing to take such emergency decisions on your behalf, but we need your consent for this.

    You may either:

    1) Download a Form of Undertaking and then the completed form of undertaking below

    OR

    2) Indicate below that you authorise the College Nurse and/or Heads of Wellbeing to take decisions on your behalf in case of serious ill-health or emergency
