This online form and embedded documents relate to and contain important information about your admissions to LMH, your induction to Oxford, regulations, as well as other resources you will find helpful during your time at LMH.

Please complete the following questionnaire within ONE week of receiving it. This form will take 30-60 minutes to complete.

Please read this form carefully and thoroughly. If you do not complete this form, you will not be able to enrol as a new student.

We will ensure that any personal data as defined under the General Data Protection Regulation relating to you is processed and stored in a way that complies with the law. College parenting scheme

New graduate students have the option to indicate if they would like to be assigned a "College Parent". College Parents are current graduate students who are assigned to help you settle in and answer any general queries you may have. If you would like to have a College Parent allocated to you below, please complete the following form:

Disclosure Statement

The College aims to support students in the best possible way during their studies.  In order to do this, we will sometimes need to disclose information about your wellbeing to other professional services, your Department/Faculty, or others associated with your learning.

The people you might need us to share information with may include: Tutor for Graduates, Senior Tutor, College Advisor, Dean, Head of Wellbeing, Academic Registrar, University's Exams Office, University's Disability Advisory Service, Library Services and other wellbeing services. This list is not exhaustive and information may be shared with other professionals. Such sharing will be on a strictly need to know basis, and shared with your consent.

In very exceptional circumstances, we may take a decision to disclose information without your consent. These circumstances are:

1. If we believe that there is a serious risk to your safety or the safety of another person, we may contact University colleagues or external services such as a doctor or mental health team. We may also need to share information with your parents or guardians. Whenever possible we will discuss this with you and seek your permission to share information, but in some circumstances we may have to proceed without your consent.

2. When the member of staff/service could be liable to civil or criminal court procedure if the information is not disclosed e.g. if you informed us that you were involved in criminal activity.

Any information that is shared is kept to a minimum, and shared only on a ‘need to know’ basis. A record of the disclosure will be retained as part of your file which you are entitled to see in accordance with the provisions of the General Data Protection Regulation (GDPR).  The College’s Data Protection Policies and Privacy Notices are available on the LMH website here: https://www.lmh.ox.ac.uk/about-lmh/further-information/privacy-and-data-protection
The College has a responsibility for making decisions on how my data is processed. The College will process my data for the purposes of providing support (both academic and otherwise), relating to my welfare, disability or wellbeing during my period of study. The College may share data where there is a legal, contractual or regulatory requirement to do so or in order to provide better support for my needs. This may be with organisations such as Student Loans Company, NHS Bursary, GP Surgery, Non-Medical Help practitioners, The Higher Education Statistics Agency (HESA) or others.

You must apply a 'wet' (i.e. non-typed) signature to the contract. A digital copy of your hand-written signature is acceptable.

    Form of Undertaking

    In the unlikely case of a serious ill-health or other emergency situation in which you are not able to give your own consent, we need you to let us know whom you would like to make decisions on your behalf. We need them to complete this form so that they are aware of this responsibility. We would normally try to contact the person you consider to be your next of kin wherever they live, but we do need a contact who is living in the UK. If you have no close relatives or friends in the UK who would fulfil this role for you, it is possible for you to authorise the College nurse or Head of Wellbeing to take such emergency decisions on your behalf, but we need your consent for this.

      Registration Time Slot Booking (Michaelmas Term Starters Only)

      Hilary Term starters must attend the registration session at LMH in the Olga Pocock Room, Monday 13 January, any time between 1pm-3pm. Pre-booking is not required. - please select the option below indicating you have made a bookingto be able to submit this form.


      Following completion of the University's online registration processes which opens on 1 September, you must attend College registration to enrol as a student at the University and activate the IT access you will require as a student. This IT access will also permit you to download an enrolment certificate which can be used for opening student bank accounts and for council tax purposes.

      PGCE and Master of Science in Educational Assessment registration sessions may be booked here.

      Please make a note of the date, time, and location of the time slot you book.


      IMPORTANT

      Please also ensure you complete the University's online registration process, which opens on 1 September, prior to attending your College registration session. Instructions for the University's process can be found in the Guide to Registration. If this process is not completed we will not be able to enrol you and you risk being unable to commence your study at Oxford.

      1. River

      Boating can be dangerous. Beware of deep water, mud, water weeds and strong currents. Due to the risk of Weil’s disease or other infections, avoid contact with river water wherever possible. Do not swim in the river.

      2. Trees

      Many of our trees are quite old. During periods of high wind trees may become unstable and large branches have been known to fall down. Avoid walking under trees in windy or stormy weather.

      3. Electrical Substation

      The electrical substation for the College and surrounding streets is located within the College grounds. It is housed in a locked brick building in Norham Mews, marked with yellow signs. Extremely high voltages are present. Under no circumstances should you enter the building. If you notice that the building is unlocked please inform the Porters’ Lodge immediately.

      I accept that in entering the river or using the College boats I do so entirely at my own risk. I accept that using the grounds in high winds is entirely at my own risk. I accept that entering the electrical substation located on the College grounds is not permitted under any circumstances. I agree that I shall not have, or make any claim for loss of life, injury, or other loss or damage to property, whether caused by negligence or otherwise, against Lady Margaret Hall, or any employee of Lady Margaret Hall, for any incident caused by or due to 1 – 3 above.

      Medical Registration

      All new students are asked to complete an online registration process for the medical practice used by LMH, Banbury Road Medical Centre.  Please follow the instructions in the Medical Registration Guide.

      ID and Visa Upload

      All students

      As part of the student registration process, we will need to verify your identity before we can enrol you and issue your student card. Please provide a form of government-issued ID (such as a passport or driving licence) which we will store and use to compare against when you register with the College.


      International students from outside EU/EEA/Switzerland

      If you are an international student attending the University on a student or other visa, you will also need to provide:

      • A scan of the photo page of your passport
      • A scan of your visa vignette sticker in your passport if you have been issued one
      • A scan of both sides of your Biometric Residence Permit (BRP) (showing that the visa will cover the dates for which you will be studying your upcoming course) OR an immigration status share code (https://www.gov.uk/view-prove-immigration-status)
      • If you currently hold a student visa having studied at either Oxford or another instiution and have not yet been issued the visa which you will use for your upcoming studies, in addition to the above, please also send evidence of a submitted new visa application

      If you are expecting to receive a Biometric Residence Permit or generate a status share code but are not yet able to provide this, please ensure you send a copy to graduate.admissions@lmh.ox.ac.uk before attending for your in-person registration session.


      EU/EEA/Switzerland students holding settled status, pre-settled status, or a student visa

      If you are an EU/EEA/Switzerland student who holds Settled or Pre-Settled Status, or has successfully applied for a student visa (which will cover the dates in which you will be studying for your upcoming course), you will also need to provide:


      We will be asking all students arriving on a student visa their arrival date in the UK at in-person registration to ensure the date of arrival occured after the visa became valid.

        Online Consent Course

        Ahead of your arrival in Oxford, you are required to attend an online training session about consent.  The course can be accessed on the University website after your University Card Form has been received and processed and your University IT account details have been issued. Please visit the Online Consent Course to bookmark the page to access it later.  This course is compulsory even if you have done something similar in the past.

        Graduate Freshers' Dinner (Michaelmas Term Starters Only)

        With apologies to Hilary term starters, a Freshers' Dinner will not be available. Please select 'No, I cannot attend the Graduate Freshers' Dinner' below.


        You are warmly invited to the Graduate Freshers' Dinner; the date is to be confirmed but this will usually be on the Tuesday 15 October, with pre-dinner drinks taking place in the Monson Room from 6.45pm, followed by dinner in the Dining Hall from 7.15pm.

        You must RSVP if you wish to attend*. Guests are not permitted to attend. 

        Dress code: Smart (e.g. suits or equivalent, no academic gowns).

        This dinner is free of charge. 

        *Priority for places will be given to full-time students.
